1. Services Provided

 

 

Liens 2 Go, LLC (“Company”) provides administrative lien and lien release preparation services. By using our Services, you acknowledge and agree that:

 

• We act only as a limited agent for preparing and filing lien documents.

• We do not provide legal advice, review, or modify any submitted documents.

• We will file and/or mail exactly what you submit and only to you, the client.

• You are responsible for ensuring accuracy, completeness, and compliance with all statutory requirements, including filing deadlines and service to required parties (patients, insurers, tortfeasors, etc.).

 

 

 

 

 

2. Payment Terms

 

 

• You agree to submit payment upon request for Services rendered.

• Processing and filing will only begin once payment is received.

• Delayed payments may result in delayed filing.

 

 

 

 

 

3. Client Responsibilities

 

 

You, as a healthcare provider, hospital, or authorized agent, are solely responsible for:

 

• Determining eligibility for liens or lien releases under Arizona law (A.R.S. § 33-931 et seq. and A.R.S. § 47-9101 et seq.).

• Providing accurate and complete information in all submissions.

• Serving lien documents to all other required parties beyond delivery to the client.

• Maintaining records of submissions and mailing.

 

 

 

 

 

4. Passive Filing & Accuracy Disclaimer

 

 

• Liens 2 Go, LLC files documents exactly as submitted; we will not edit, verify, or correct information.

• We are not responsible for errors, omissions, or statutory compliance issues in any submitted document.

• Our responsibility ends once the document is delivered to you.

 

 

 

 

 

5. Data Protection and Confidentiality

 

 

• We handle all patient and client information (“Confidential Information”) in accordance with applicable privacy laws, including HIPAA where applicable.

• Confidential Information is used solely to prepare and file lien documents.

• You are responsible for ensuring that all information provided complies with privacy laws and is accurate.

• We are not liable for unauthorized access, disclosure, or breaches beyond our reasonable control.

• We maintain reasonable safeguards to protect Confidential Information.

 

 

 

 

 

6. Indemnification

 

 

You agree to indemnify and hold harmless Liens 2 Go, LLC, its owners, officers, employees, and agents from any claims, damages, or expenses arising from:

 

• Your submission of inaccurate, incomplete, or unlawful information.

• Your failure to serve lien documents or comply with statutory requirements.

• Any claim or dispute arising from the use of the Services.

 

 

 

 

 

7. Limitation of Liability

 

 

• We are not liable for direct or indirect damages arising from your use of our Services, including lost revenue, lost lien rights, or errors in documents submitted by you.

• Liability for any claim is limited to the amount paid for the specific Service at issue.

 

 

 

 

 

8. Electronic Communications and Signatures

 

 

• By submitting forms or communicating via our website, you consent to electronic communications.

• Electronic signatures, submissions, and transactions are legally binding.

 

 

 

 

 

9. Governing Law & Dispute Resolution

 

 

• These Terms are governed by the laws of the State of Arizona.

• Any disputes should first be addressed through informal negotiations before pursuing legal remedies.

 

 

 

 

 

10. Miscellaneous

 

 

• These Terms constitute the entire agreement between you and Liens 2 Go, LLC.

• If any provision is invalid or unenforceable, the remainder of the Terms remains in effect.

• No joint venture, partnership, or employment relationship is created by these Terms.
